Meiji Gakuin University Workplace (On-campus) Vaccination Program


Meiji Gakuin University will offer the COVID-19 vaccine as a workplace (on-campus) vaccination program.
Please refer to the following for details on the vaccination and how to make a reservation.

*The Meiji Gakuin University Workplace Vaccination Program offers the vaccine to those who wish to be vaccinated.
The vaccination is optional,not mandatory. Whether or not to receive it is up to each individual. Those who decide not to receive a vaccination will not be penalized for that decision. Each individual is to understand the benefits of infection prevention and risks of adverse reactions of the vaccination, and receive it at their own discretion.  

  
  
 
 
  
     

1.Dates and times

     

First dose: From September 4 (Sat) to September 5 (Sun), 9 a.m. to 5 p.m.
Second dose: From October 2 (Sat) to October 3 (Mon), 9 a.m. to 5 p.m.
*The above schedule may be delayed depending on vaccine availability.

  
 
 
  
     

2.Location

     
Pallet Zone Shirokane, Shirokane campus, Meiji Gakuin University map  
*You will need to receive a total of two doses. In the Meiji Gakuin University Workplace Vaccination Program, both doses must be received on the same campus. Those who wish to receive only one of the two doses on campus are not eligible for the Workplace Vaccination Program.

4.The vaccine

      
Takeda/Moderna COVID-19 Vaccine will be used. Before you receive the vaccine, please read the information contained in the website shown below to learn about precautions before and after vaccination, and how to respond in the event of an adverse reaction.

5.Making a reservation

    
You must make a reservation to be vaccinated under the Meiji Gakuin University Workplace Vaccination Program.
(You do not need a vaccination voucher from your local municipality to make a reservation.)
You can make a reservation via the following website. (The website will be scheduled to begin accepting reservations from 10 a.m on August 25 (Wed) to 12 p.m on Septembet 1 (Wed).)

*The above reservation schedule may be delayed depending on vaccine availability.
*DO NOT under any circumstances make a reservation for both the university and municipality vaccination programs.
*Plan your schedule so that you can spend several days recuperating at home after the vaccination in the event of an adverse reaction.
  
 
 
  
    

6.What to bring on vaccination day

    
①The original vaccination voucher from your municipality (if you have not yet received one, bring the “Record of Vaccination for COVID-19” as below).

The vaccination voucher comes with a Certificate of Vaccination. Bring the vaccination voucher as is. Do not peel off the sticker on the voucher. You can receive a workplace vaccination at the university even if you have not yet received your vaccination voucher, but you should bring a printout of the “Record of Vaccination for COVID-19” filled out with the necessary information. If you receive your vaccination voucher from your municipality before your second dose, bring the voucher to the venue when you come for your second dose. If your vaccination voucher does not arrive by the date of your second dose, you are asked to submit it to the university as soon as it arrives.

②Prevaccination Screening Questionnaire for COVID-19 vaccine
You will need to fill in one sheet for each dose. If you have not yet received a Prevaccination Screening Questionnaire for COVID-19 vaccine from your municipality, bring a printout of the Questionnaire filled out with the necessary information.

The English version of the Prevaccination Screening Questionnaire is available here.
However, one may not submit the English version. Please refer to this English version to answer and submit the Japanese version. It is acceptable to use Roman letters (alphabet) to fill in your name and address on the Questionnaire, but make sure you use Japanese kana to fill in the “Furigana” field.

③Personal ID
Please bring your student card, staff card, or other proof of your affiliation. (You may be asked to submit proof of identity. Please bring your health insurance card, My Number Card, driver’s license or passport, etc.)

  
 
 
  
    

7.What to wear

     
The vaccine is injected into the upper arm muscle. Please wear clothes that make it easy to access your shoulders (see photo).
          

  
 
 
  
    

8.Other precautions for vaccination day

    
• You cannot receive the vaccine if your body temperature is 37.5°C or higher. Please refrain from coming to the vaccination venue.
• We expect significant crowding at the venue. Please do not come to the venue more than 10 minutes in advance of your appointment time.
• You must wear a mask (non-woven mask recommended) on the day of vaccination.
• After receiving the vaccination, you will be asked to remain in the monitoring area for at least 15 minutes (about 30 minutes for individuals who experienced allergic reactions to vaccines in the past, or who have a history of anaphylaxis).
• If you experience an abnormal reaction at the injection site, or a change in your physical condition, immediately notify the vaccination staff nearby.
• You may develop a fever after the vaccination. Have some pain relief medication ready in case this occurs. You will not be given such medication at the vaccination venue. Please purchase such medicines at your neighborhood drugstore or have some prescribed by your doctor.
